IMPORTANT HEALTH COVERAGE TAX DOCUMENTS
Due to new regulations by the government, 1095-C forms will not be mailed out but are available upon request. If you need to request a 1095-C form, please contact the Benefits Department either by email at [email protected], phone at 602-283-9224, or via mail by sending a written request to 3636 N Central Ave, Phoenix, AZ 85012 (Attn: Benefits Department). You can also request the form by reaching out to your local HR team. Once the request is made, Cavco will have 30 days to provide the form.
Additionally, per IRS guidelines, you no longer need the form to provide proof of coverage. One of the below listed documents will suffice.
“You do not have to wait for Form 1095-B or 1095-C from your coverage provider or employer to file your individual income tax return. You can use alternative documentation to prepare your tax return. Acceptable forms of documentation include:
- Insurance cards
- Explanation of benefits
- Statements from your insurer
- W-2 or payroll statements showing health insurance deductions
- Records of advance premium tax credit payments
- Other statements showing that you or a family member had health coverage
You do not need to send the IRS proof of your health coverage, but you should keep the documentation with your other tax records, including records of employer-provided coverage, premiums paid, and the type of coverage.”
